Request for Data

The Cancer Alliance Queensland request for data involves two-steps:

Step 1. Download and complete the Cancer Alliance Queensland Data Request Form.
This form outlines the specifics of your data request (eg Type of data request, purpose, data elements required).

Step 2. Fill out the online registration for data request below.
This is where you will upload your completed Data Request Form and supporting information to complete the submission process.

The Data Request Form must be uploaded to submit the registration. You can load up to 3 files.

The Cancer Alliance Queensland Cancer Control Information team aims to provide a timely response to requests for data. A Cancer Control Information team member will contact you regarding your request.
